Patient Assessment and Risk Analysis Workshop

This workshop is recommended prior to implementing the TrendCare Assessment function or for any site requiring re-training in the Assessment function due to the turnover of key personnel. The Patient Assessment function enables an organisation to author its own set of patient history assessments, outcome assessments and risk assessments in the TrendCare system, with all assessment revisions being recorded as they are released and retained when superseded. Comprehensive risk assessment reporting is available for all assessments.

Workshop Description

A one-day, comprehensive workshop providing users with the skills to develop, update, maintain and manage patient assessments within the TrendCare system.  The process of auditing assessments and the reporting capability of the assessment module is also covered in detail in this workshop. Assessments can be developed in Pre-Admission, Inpatient or Post Discharge phases of a patient’s care, with risk scores and an action list being generated in response to identified risks.

Learning Outcomes

• An ability to develop, use and maintain banks of patient history assessments, risk assessments and outcome assessments in the TrendCare system.

• An ability to build assessments with multiple responses to questions and automated scoring of responses.

• To understand how to build an assessment so that an action plan is generated automatically according to the selected responses to the assessment questions.

• An ability to complete multiple assessments on patients in pre-admission and/or during their inpatient episode of care and post discharge phase of care.

• An ability to customise repeated assessments with preselected responses during a patient’s episode of care.

• To understand how to use audits to identify incomplete and omitted assessments.

• An ability to interpret assessment reports in the TrendCare system.
