Clinical Pathway (Care Path) Development Workshop

This workshop is recommended prior to using the TrendCare Care Path function or for any site requiring re-training in the Care Path function due to the turnover of key personnel.  The Care Path function, when effectively implemented, can reduce length of stay, minimise episode of care cost and improve patient outcomes.

Pre-requisite Presentation

A 1-hour presentation on the concepts, benefits and processes required to implement care paths successfully in an acute facility.

Workshop Description

A one-day training workshop covers the maintenance setup, the development and the use of care paths in a clinical department. TrendCare Care Paths incorporate an organisation’s best practice care guidelines for specific procedures or diagnoses.  The focus is on incorporating the optimal sequencing and timing of all interventions provided by care givers including medical, nursing and allied health during a patient’s continuum of care.

Learning Outcomes

• An ability to setup and maintain the Care Path maintenance screens.

• An ability to develop and maintain a bank of multiple care paths in the TrendCare system.

• An ability to add a care path to a patient episode and to customise actions and expected outcomes on the Care Path.

• An ability to select the reason for variances to care path actions and expected outcomes.

• An ability to develop a care path, which can be given to a patient prior to admission, as a guide outlining their care and progress during their episode of care.

• An ability to understand the suite of care path reports that are available that track and trend variances to care path actions and expected outcomes.

Workshop Delivery Method

The workshop is best delivered on site, however an option to deliver remotely would be considered/negotiated if sufficient scoping work was completed between the TrendCare consultants and the organisation beforehand.
